Pharmacological action
Capsaicin mainly exerts analgesic and antipruritic effects by affecting the release and storage of neuropeptide substance P. Substance P is a type of eleven peptide, widely distributed in the afferent sensory nerve fibers, posterior root ganglia, and posterior horn of the spinal cord. It is an important neurotransmitter that can transmit pain and itching from the peripheral nerves to the spinal cord and superior central nervous system. Capsaicin mainly acts on substance P on C-type sensory neurons, and it is some unmyelinated slow conduction fibers in C-type nerve fibers that transmit skin pain and pathological itching. Local topical application of capsaicin acts on peripheral nerve axons, resulting in a decrease in substance P from all neurons (peripheral and central). At the beginning, axonal conduction is blocked, followed by a decrease in substance P synthesis. Numerous experiments have shown that in response to harmful stimuli, sensory hypersensitivity caused by the release of substance P can prevent certain types of pain and itching when medication is used to reduce local substance P.
